Bir ayak replantasyon olgusunda pes kavus deformitesinin onarımı

Abstract

Pes kavus, ayak ark yüksekliğinin artması, ayak parmaklarının hiperekstansiyonu ile karakterterize ve ayakta pençe görünümüne neden olan bir deformitedir. Ellidört yaşında erkek hasta ayağını çay biçme motoruna kaptırma sonucu total ayak amputasyonu nedeniyle acil servisimize başvurdu. Hastanın yapılan fizik muayenesinde metotarso-falangeal eklem hizasından sağ ayağının total ampute olduğu ve sağlam ayağında idiyopatik kökenli pes kavus deformitesinin olduğu tespit edildi. Bu çalışmada, ekstremite replantasyonlarında ilk basamak olan kemik stabilizasyonu ve tendon onarımında, bu deformitenin varlığında yaşanan güçlükler ve mevcut tedavi yaklaşımları gözden geçirilerek, başarı ile neticelenen olguda uygulanan yeni cerrahi tekniğin sonuçlarının bildirilmesi amaçlanmıştır

Pes cavus is a deformity of the foot characterized by an abnormally increased arch and hyperextension of the toes that gives the foot the appearance of a claw. We encountered a case of a 54-year-old male patient who presented with total right foot amputation due to injury with the tea harvesting engine. During physical examination, we observed a right foot amputation in the level of the metatarsophalangeal area and a bilateral idiopathic pes cavus deformity. In this unusual case, we aimed to report the difficulties experienced in bone stabilization and tendon repair, which are the first steps in limb replantation, and the tendon lengthening technique used to correct the pes cavus deformity, which is a novel technique for this deformity, along with a review of the related literature.
